Reporting an Injury

You can use the web-based claims reporting system called Visual Liquid Web (VLW) that includes two options for submitting claims. First, a designated person (named user with a password) can submit a claim electronically to MCI. A second option involves a field supervisor or manager (an anonymous user who must have the agency location code) completing the first portion of the claim and then submitting this to the named user who completes and submits the claim. The human resource representative receives the claim number as acknowledgment of receipt.

1. Report a claim by clicking either of the links below to access the reporting system:

2. Include information on the FROI submission regarding the accident description and return-to-work.

3. Submit the supplemental reports when the employee returns to work or is out of work. See the Forms Library page for a list of forms to use.

4. Complete a wage chart for all lost-time claims.
